Primary care montana (PCMT) - tiers 1 and 2 - launching july 1, 2026

Technical Assistance: Beginning in April 2026, DPHHS will provide technical assistance to support providers during the “go-live” period.

The program will be rolled out in phases to ensure a smooth transition: 

  • Tiers 1 and 2 (Effective July 1, 2026): Focus on improving quality metrics, increasing post-hospitalization follow-ups, and reducing readmissions. 
  • Tier 3 (Targeted July 2027): This advanced tier will focus on high-risk care management and offer opportunities for shared savings.

MTAAP is actively participating in DPHHS stakeholder meetings as part of the Primary Care Collaborative, The Primary Care Collaborative (PCC) is a multistakeholder group that organized in fall 2023 to inform Montana Medicaid’s Primary Care Case Management (PCCM) program redesign and elevate the collective voice of primary care practices to advocate for policies and programs that support access to high-quality primary care in every Montana community.

As part of the PCC, MTAAP has provided ongoing program design input, including anticipated barriers and challenges, on behalf of MTAAP members over the past year and a half. If you would like to be more closely involved in this program redesign, please contact ed@mtpeds.org.
